Not sure about most women, but my wife is very anal phobic. She’s always worried about getting an infection. She’s prone to getting UTIs, so that’s made her extremely anal phobic. I think it is all about the approach to anal sex. It has to be taken slowly with lots of lube and finger play or appropriate thin toys to start before eventually progressing to fucking.
